Case Management
For persons who are prone to relapse,
that need ongoing case management on an out-patient basis.
For persons who need more intense
treatment, there is a three month program which offers 20 hours
of services per week.
Rule 25 funded or affordable self-pay.
A Rule 43 Program.

- Inability to abstain from chemical
use without further supportive programming and case management
- Loss and impairment of employment
due to usage, or
- Loss of education due to usage,

- Lack of family support, or

Admission Hours
9:00 a.m. - 4:00 p.m., Monday - Friday
Average Length of Program
8 hours per week for 10 weeks
4 hours per week for 20 weeks
20 hours per week for 90 days
Can be tailor-made to fit clients
schedule and needs.

Continuum of Care Program
Services provide for specialized care
of adults suffering from continuous patterns of relapse and dysfunction
due to the use of alcohol and drugs. Those who need Out-Patient
case management for an ongoing period of time.
Program Includes:
- Group Sessions
- Individual Counseling
- Teaching Sessions
- Relapse with random U.A.'s and
breathalyzers
- AA group

- Ministerial Counseling

Qualifications:
Those adult males and females who
are Rule 25 approved, private pay, or have insurance. Prospective
clients must have completed primary treatment and Halfway House
within the last 5 years.
Persons who qualify for halfway house
but have suitable living arrangements also would be accepted as
Out-Patient.
Process
Step 1:
Call Transformation House Intake Coordinator
for Post-Primary Treatment Program, Out-Patient Halfway House Assessment.
All types of referrals are accepted.
Step 2:
Initial Interview is scheduled and
upon staff and client's agreement a start date is determined.
Step 3:
Individual Program Developed

who cannot stay sober without a structured program.
This program is tailored to meet each
individual's needs.
